A young woman with thrombocytosis and a right atrial thrombus was presented after a repeated episode of pulmonary embolism. Chronic thromboembolic pulmonary hypertension was suspected, based on a severely increased systolic pulmonary arterial pressure and the diagnosis was confirmed by pulmonary angiography. Multidisciplinary approach and antithrombotic therapy associated with specific pulmonary arterial hypertension treatment was beneficial for this patient deemed to be inoperable for pulmonary endarterectomy and atrial thrombus excision.
